Take it apart, clean it, regrease it, and put it back together. If that doesn't work, nuke it!
if you have good HS tension and your stem is tight, the top bolt isnt really necessary.
how would the fork fall off or the HT/HS get destroyed if the stem is tight?

Folks ride flat all the time without the top bolt, Nesquik. You compress the headset first with the stem and top bolt, tighten the stem, then just remove the bolt. It'll loosen over a long enough amount of riding, but it won't straight fly off and maim someone. That'd be interesting to see though... lol
